- Returning Families Registration Form
- Please note: This form is for families who have been at Landmark for a year or more. Only one form needed per family. Please fill out this form online, print it out, and bring it to Registration on August 8.
- New Families Registration Form
- Please note: This form is for families who will be new to Landmark in 2008-2009. Only one form needed per family. Please fill out this form online, print it out, and bring it to Registration on August 8.
- Emergency Medical Form
- Please note: One form is required for each Landmark student. Please fill out this form online, print it out and bring it to Registration on August 8.
- Field Trip Release Form
- Please note: One form is required for each Landmark student. Please fill out this form online, print it out and bring it to Registration on August 8.
- After School Program Registration Form
- Please note: This form is required for all Elementary families. You may fill the form out online, print it out, and bring it to registration on August 8.
Optional Forms
- Parent Questionnaire on Cafeteria Food Service
- This optional form provides important feedback to help improve our cafeteria food service. You may fill the form out online, print it out and bring to registration on August 8.
- Permission to Pick Up
- This optional form may be completed by any family who needs to specify who may and may not pick up their child. You may fill the form out online, print it out, and bring it to the Transportation table at Registration on August 8. You may also bring the form into the elementary office anytime after August 8.
- Sports Physical Form
- All Landmark athletes must have an up-to-date sports physical form on file, prior to beginning practice or play.
- Student Parking Permits
- This optional form should be completed for any upper school student who would like a permit to park on campus. You may fill the form out online, print it out, and bring it to the Transportation table at Registration on August 8.
